What should you know about unmethylated glioblastoma?

Glioblastoma is a fast-growing brain tumour. The MGMT promoter is a region of genetic material that can affect how a tumour repairs damage from some chemotherapy medicines. A laboratory test classifies it as methylated or unmethylated. An unmethylated result is one part of the tumour profile; it does not describe your symptoms, your strength, or your worth of treatment.

Your team combines this result with the tumour’s location, how much can be removed safely, scan findings, general health, and your wishes. The result may influence how they explain expected treatment effects, but it does not by itself predict exactly what will happen to one person.

Ask for the pathology report to be explained in plain language and ask whether the test was performed on the current tumour sample. Discuss surgery, radiotherapy, and chemotherapy in the context of your scan and recovery. Brain tumour surgery may provide tissue for testing as well as relieve pressure when appropriate. Bring a medicines list and a trusted person to consultations; treatment planning can involve several specialties.

Ask whether a clinical trial is suitable for your diagnosis, location, and current treatment stage. A trial is optional, and the team should explain its purpose, visits, possible burdens, and alternatives before you decide.

Contact your oncology or neurosurgery team promptly for a new or worsening seizure, increasing confusion, a severe or rapidly worsening headache, repeated vomiting, new weakness or numbness, trouble speaking, or a marked change in vision. Seek emergency help for a seizure that does not stop, loss of consciousness, or sudden severe neurological change. These symptoms need assessment because swelling, bleeding, medication effects, or tumour-related pressure can require urgent treatment.
