What the pain may indicate

A twist can stretch wrist ligaments or irritate tendons that guide the hand and thumb. Pain may be worse when gripping, turning a key, pushing up from a chair, or moving the thumb. A fall onto the hand can also injure the distal radius, even when bruising is modest. The exact tender spot and the hand's strength and movement are useful clues.

Not every painful wrist problem starts with a single injury. De Quervain's tenosynovitis can cause pain on the thumb side, while carpal tunnel syndrome more often brings tingling or numbness in the hand. Hand osteoarthritis may cause activity-related aching and stiffness.

Protect the wrist

Stop movements that sharply increase pain and remove rings if swelling may develop. Keep the wrist in a comfortable, neutral position; a removable support can help with tasks if it does not cause pressure, tingling, or colour change. Use a wrapped cold pack briefly and keep the hand raised when resting. Avoid forceful gripping and lifting until movement is easier.

Gentle finger movement helps prevent stiffness, but do not force wrist rotation or use strengthening exercises while pain is acute. A clinician can advise when to start motion and whether an examination or imaging is needed.

When to seek care

Get urgent help for a visibly deformed wrist, an open wound near the injury, severe pain, loss of feeling, or fingers that become pale, blue, or cold. Arrange prompt review if you cannot grip, move the wrist normally, or use the hand for routine tasks. Persistent focal tenderness after a fall should also be assessed because some fractures are not obvious from appearance alone.
