Top of hand swollen and sore: what to know

Swelling on the back of the hand may appear after a fall, impact, heavy gripping, or repetitive wrist and finger movement. A fracture near the wrist can cause pain, swelling, bruising, and difficulty using the hand; a Distal Radius Fracture is one example. Warmth, redness, and worsening tenderness can point to infection or inflammation. Rheumatoid Arthritis can present with painful, swollen joints and morning stiffness, often affecting more than one area.

Notice whether the swelling is sudden or gradual, whether a particular finger or joint is affected, and whether you have numbness, colour change, fever, or reduced movement. A clinician may need to examine the wrist, tendons, joints, and circulation.

Remove rings and watches early if swelling is developing. Rest from gripping and lifting, keep the hand raised when possible, and use a wrapped cold pack for short periods after a recent injury if sensation and circulation are normal. Do not place ice directly on the skin or tightly wrap the hand. Gentle finger movement is reasonable if it does not increase pain, but do not force a deformed or severely painful joint.

Arrange an assessment if swelling does not settle, follows a fall or blow, or interferes with daily tasks. You can compare both hands and note changes in colour, temperature, movement, and pain.

Seek urgent care for a visibly crooked finger or wrist, inability to move the hand, severe pain after an injury, loss of sensation, blue or pale fingers, or rapidly increasing swelling. Same-day medical review is needed for fever, spreading redness, pus, marked warmth, or a wound or bite near the swollen area. If symptoms are mild but persistent or recurrent, book a clinical assessment rather than relying on home care alone.

Ask whether the swelling is more consistent with bone, tendon, joint, or skin involvement and whether an X-ray or other test is needed. Ask how to protect the hand, which movements are safe, and what changes should prompt urgent review. Mention any injury, new activity, wound, bite, fever, or morning stiffness.
