TNM staging in non-small cell lung cancer

TNM staging in NSCLC records the primary tumour, regional nodes, and distant spread. Doctors use these details with tumour biology, lung function and your wishes when discussing treatment.

The T category describes how large the tumour is and whether it reaches nearby tissues or separate areas in the lung. N describes cancer found in lymph nodes near the lung or in the chest. M describes spread beyond the chest to distant sites. CT or PET imaging, brain imaging when appropriate, bronchoscopy, biopsy and node sampling may all add information. The overall stage groups these observations into a practical summary.

Two people with the same broad stage can receive different recommendations because tumour mutations, symptoms, fitness and previous treatment differ. Stage may be called clinical before surgery and updated after tissue examination. Ask which version your team is discussing.

Request the exact T, N and M categories and ask what evidence supports each one. Ask whether the biopsy confirms NSCLC subtype and whether molecular testing is appropriate. Depending on the findings, options may include lung lobectomy, stereotactic body radiotherapy, immunotherapy or targeted therapy. Ask how treatment order, expected monitoring and side-effect support relate to your stage rather than choosing from a list alone.

Tell your team quickly about increasing breathlessness, chest pain, coughing blood, a fever during treatment, new confusion, a seizure, or sudden weakness or trouble speaking. Emergency care is appropriate for severe breathing difficulty, fainting, heavy bleeding or abrupt neurological symptoms. These signs need assessment regardless of the stage recorded.
