Swollen Tip of the Thumb: What to Know

The thumb tip contains the nail, skin, soft tissue, and the end joint. A crush, puncture, splinter, bite, repetitive gripping, or irritation around the nail can make it swell. Tenderness at the thumb side of the wrist and pain when moving the thumb may also occur with De Quervain's tenosynovitis, although that condition more often affects the tendon area near the wrist than the fingertip itself. Swelling with throbbing pain or pus can point to infection, while bruising or limited movement can follow injury.

Remove rings or tight coverings before swelling increases. Rest the thumb from gripping, keep the hand raised, and use a wrapped cool pack for short periods if the swelling followed a minor injury. Do not pierce a blister, cut the nail fold, or dig for a deeply embedded splinter. Keep any wound clean and covered. A clinician can check for a foreign body, infection, fracture, or tendon problem and advise whether a splint or treatment is needed.

Get urgent help if the thumb is blue, pale, cold, numb, badly deformed, or impossible to move, or if a deep cut exposes tissue. Arrange prompt assessment for rapidly increasing swelling, severe throbbing pain, pus, fever, spreading redness, or an animal or human bite. A puncture involving the joint or nail should also be assessed, even when the opening looks small.

Ask: Could this be an infection, fracture, or tendon irritation? Do I need an X-ray or drainage? How should I dress the wound? Which thumb movements are safe, and when should I return if swelling persists?
