What should you know about time of cervical cerclage?

A planned cerclage may be discussed early in pregnancy when a previous pregnancy history suggests the cervix could shorten or open too soon. An ultrasound-indicated stitch may be considered after cervical shortening is seen. An examination-indicated stitch is a different situation and may be considered when the cervix is already opening.

Cerclage is intended to support the cervix and reduce the chance of early opening in selected cases; it cannot address every cause of preterm labour. The team weighs infection, bleeding, contractions, ruptured membranes and the condition of the pregnancy before recommending it.

Ask what finding prompted the discussion, what alternatives exist, and whether monitoring with cervical-length scans is appropriate. Tell the team about contractions, fluid leakage, bleeding, fever, previous cervical procedures and medication allergies.

If a stitch is placed, follow the specific instructions about activity, intercourse, medicines and review. Keep scheduled appointments so cervical changes and symptoms can be assessed. A cerclage is usually removed later in pregnancy or earlier if labour or waters breaking makes removal necessary; your team will explain the plan.

Contact your maternity team urgently for regular tightening, pelvic pressure, fluid leakage, bleeding, fever, foul-smelling discharge or worsening pelvic pain after cerclage. These symptoms can indicate infection, membrane rupture or preterm labour and need prompt assessment.
