Can thyroid cancer occur without a lump?

A thyroid growth may be small, deep in the gland, or positioned where it is not easy to notice by touch. Some thyroid cancers are found during imaging or an examination for another reason. Other people notice pressure in the lower neck, a change in swallowing, or a voice that stays hoarse. A symptom alone cannot identify its cause; thyroid enlargement, inflammation and non-cancerous nodules can cause similar changes.

Assessment commonly brings together a neck examination, thyroid blood tests and an ultrasound. If the ultrasound shows a finding that needs closer review, a clinician may recommend a needle sample. The result guides whether observation, surgery or another treatment is appropriate.

Arrange a medical appointment if a neck or voice change persists, returns repeatedly, or is becoming more noticeable. Write down when it began, whether swallowing solids or liquids is affected, and any history of thyroid disease or neck treatment. Bring a list of medicines and previous imaging reports if available. Do not wait to feel a lump before seeking an assessment.

A confirmed diagnosis may be managed with thyroid removal surgery, radioactive iodine therapy, targeted therapy, active monitoring or a combination, depending on the tumour type and extent. Treatment decisions are individual and may change after imaging or tissue results.

Seek urgent medical help for new or worsening difficulty breathing, inability to swallow fluids, or rapidly increasing neck swelling. Contact a clinician promptly for persistent hoarseness, repeated choking, or a new feeling of pressure in the neck. These symptoms have several possible causes, but they should not be dismissed because there is no palpable lump.
