What a twist can injure

A sudden twist can stretch a ligament around the thumb joint, irritate a tendon or damage the joint surface. A fracture or partial dislocation can also follow twisting, especially when there is swelling, bruising or a change in shape. Pain with pinch may indicate reduced stability, while pain along the thumb-side wrist during repeated movement can occur with De Quervain's Tenosynovitis. The force of the event and your ability to use the thumb are more useful clues than pain alone.

Immediate steps

Stop the activity and support the thumb in the position that feels most comfortable. Use a cloth-wrapped cold pack briefly and keep the hand raised if it is swollen. Avoid testing the joint repeatedly, forceful stretching and lifting. Do not tightly tape a thumb if it causes tingling or colour change. A clinician can assess stability and movement; a splint may be recommended while injured tissue settles. Resume light use gradually only when ordinary movement is comfortable.

When to be seen

Seek urgent care if the thumb is crooked, locked, numb, cold or discoloured, or if you cannot move it or hold a light object. Prompt assessment is appropriate after a strong twist with marked swelling, bruising or a popping sensation. Arrange review if pain, weakness or instability persists, because a ligament injury can need protection even when the thumb looks normal. An open wound or rapidly increasing swelling also needs timely attention.
