What a thumb sprain means

A sprain is an overstretched or torn ligament, the strong tissue that helps keep a thumb joint aligned. It can happen when the thumb is forced backward or sideways, including during a fall, ball injury or awkward grip. Pain, tenderness, swelling and bruising may appear around the joint. A severe sprain can make pinching or gripping feel weak, and a fracture or joint displacement can feel similar, so appearance alone cannot confirm the injury.

What you can do

Stop activities that increase pain and keep the thumb supported in a neutral, comfortable position. A wrapped cold pack can be used for short periods with a cloth barrier; do not place ice directly on skin. Keep the hand raised when swelling is noticeable. A splint may limit unwanted movement, but it should not cause numbness, colour change or increasing throbbing. Once pain allows, gently move the thumb within a comfortable range so it does not become stiff. A clinician or physiotherapist can guide strengthening and return to sport or work; physiotherapy may be useful when grip remains weak.

Arrange prompt assessment after a forceful injury if the thumb looks crooked, feels unstable, cannot bend or straighten, or you cannot pinch gently. Seek urgent care for a pale, blue, cold or numb thumb, severe swelling, an open wound, or pain that is rapidly worsening. Even a milder injury deserves review if swelling and movement do not begin to improve, because an untreated ligament tear can affect pinch stability. A clinician may examine the joint and decide whether imaging or more protective support is needed.
