Thumb Spica Cast for a Scaphoid Fracture

The scaphoid is a small wrist bone near the base of the thumb. A thumb spica cast extends around the wrist and includes the thumb, limiting movements that could disturb the injured bone. The exact cast design depends on the fracture's position, stability, swelling and examination findings.

Pain or tenderness in the thumb-side wrist can occur with a scaphoid fracture, but wrist pain has other causes too. Early X-rays may not show every fracture clearly, so your clinician may arrange repeat imaging or another scan when symptoms and examination remain concerning.

Keep the cast dry and never place objects inside it to scratch. Rest the hand above heart level when swelling is present, move the free fingers regularly, and follow the prescribed restrictions on lifting and driving. Do not trim, loosen or remove the cast yourself. Keep follow-up appointments so the team can check the cast, circulation and healing.

If the cast becomes wet, cracked, loose, tight or uncomfortable, contact the fracture service. You may need a cast adjustment or replacement. Rehabilitation for wrist and thumb stiffness usually begins when your clinician says the fracture is ready for movement.

Seek urgent help if fingers become numb, unusually cold, blue or pale, or if pain and swelling increase despite elevation. Contact the care team promptly for burning, pressure, foul odor, drainage, a damaged cast or inability to move the fingers. A cast that feels tight after swelling changes needs timely review.
