Surface pain versus deeper pain

Dry, cracked or scraped skin is often tender when touched and may sting with soap or water. Pain beside the nail can follow a hangnail or irritated cuticle; redness, warmth, swelling or drainage raises concern for infection. A blister or pressure spot may hurt at the contact area. If the skin looks normal but gripping or thumb motion causes pain, a tendon or joint may be involved instead.

Pain along the thumb-side wrist can occur with De Quervain's Tenosynovitis, which affects tendons rather than the skin. Avoid assuming the source from appearance alone if symptoms persist or movement becomes limited.

Simple skin care

Wash gently, pat dry and cover a small break in the skin with a clean dressing. Avoid picking the cuticle, biting the nail or using harsh products on the sore area. Moisturiser can help intact dry skin, but do not put products into a deep or contaminated wound without advice. Reduce pressure and friction while the area heals.

A clinician or pharmacist can assess worsening nail-fold pain, a spreading rash or a wound that is not settling. They can also check whether pain is actually coming from a tendon or joint.

When to get help

Seek prompt care for spreading redness, increasing warmth, pus, fever, red streaks, severe swelling or pain out of proportion to the visible skin change. Urgent assessment is needed if the thumb becomes numb, pale or blue, or if a wound follows a bite, puncture or crush injury.
