Understanding the location
The thumb has a proximal phalanx near the hand and a distal phalanx near the nail. Pain close to a joint may be linked to the joint capsule or nearby tendon, while pain directly over a bone is more concerning after a knock, crush or forceful bend. A tender fingertip or nail edge can instead reflect skin irritation.
Look for swelling, bruising, a change in alignment, clicking, reduced bending or pain when pressing along the bone. De Quervain's Tenosynovitis usually involves tendons nearer the thumb-side wrist, so the exact location helps separate that pattern from pain in a phalanx.
Protecting the thumb
Stop activities that cause sharp pain and remove rings if swelling is developing. Keep the thumb supported in a comfortable position, but do not force it straight or tightly tape it. Elevate the hand if it is swollen. A pharmacist or clinician can advise on suitable pain relief and whether a temporary support is appropriate.
Arrange an examination when bone tenderness, swelling, bruising or restricted movement continues. An assessment can compare alignment and strength and determine whether an X-ray or another test is needed.
Warning signs
Seek urgent care for deformity, an open wound, severe swelling, numbness, blue or pale color, or inability to bend or straighten the thumb. Prompt review is also needed for increasing warmth and redness, drainage or fever, especially around the nail or a wound.
