Thumb pain with a weak grip
Thumb pain that prevents a firm grip should be assessed, particularly after injury or if weakness is increasing. Pain may cause you to release objects even when the muscles are intact, while tendon, joint or nerve problems can reduce force more directly. The exact location and what you cannot do are useful to record.
Understanding the gripping problem
Difficulty opening a jar, turning a key or pinching may point to pain around the thumb base or wrist-side tendons. De Quervain's tenosynovitis can present with pain in that thumb-side area during thumb and wrist movement. Bruising, swelling or tenderness after a knock raises concern for an injury. Tingling, numbness or weakness in several fingers may suggest nerve involvement. Thumb pain with reduced grip does not by itself establish a single condition.
Steps to take
Stop forceful gripping and pinching for now, and use two hands or larger handles for necessary tasks. Keep the wrist as straight as comfortable and move the thumb gently within a pain-limited range. Ice wrapped in cloth can be used briefly after aggravating activity. Do not use a tight support if it causes numbness or colour change. A clinician can check strength, sensation, movement and injury-related tenderness.
When it needs prompt care
Get urgent care for a deformed thumb, an open wound, a thumb that becomes cold or discoloured, or sudden inability to move it after trauma. Seek prompt review for worsening weakness, persistent numbness, severe swelling, spreading redness or fever. If grip remains limited without a clear injury, arrange an appointment rather than continuing the provoking activity.
Useful questions
Is my grip limited by pain or by weakness? Which tendon, joint or nerve should be examined? Should I use a temporary thumb brace? When can I safely resume gripping tasks?

Questions people ask
What if I can move the thumb but cannot grip?
Pain inhibition, tendon irritation, joint strain and nerve symptoms can all affect grip; an examination helps distinguish them.
Can I exercise the hand?
Gentle, comfortable movement may be reasonable, but stop exercises that increase pain or weakness and seek assessment if function is declining.
