Throbbing Pain Under the Thumb Nail

Throbbing pain under the thumbnail is often a local nail or fingertip problem, but injury and infection need careful attention. A recent crush or knock can cause tenderness and pressure beneath the nail. Tight pressure, a small splinter, or irritation at the nail edge can feel similar. Look for colour change beneath the nail, swelling, warmth, drainage, or a break in the skin.

Pain directly beneath the nail points more toward the nail bed or fingertip than the thumb joint. Redness and swelling beside the nail can occur with inflammation or infection. Pain at the thumb base or wrist has a different pattern; pain there with gripping or thumb motion may occur with De Quervain's Tenosynovitis. A clinician can examine the nail, skin, circulation, and bone when the cause is unclear.

Protect the thumb from further pressure and keep the nail clean and dry. If an injury caused swelling, use a wrapped cool pack briefly and keep the hand raised when comfortable. Do not drill, pierce, or lift the nail yourself, and do not dig into the nail fold to remove a suspected splinter. Seek an assessment if the pain is strong, worsening, or not settling.

Get prompt care after a crush injury with severe pressure, a loose or split nail, an open wound, or difficulty bending the thumb. Fever, pus, spreading redness, increasing warmth, or red streaking can signal infection. Immediate help is needed if the thumb becomes blue, pale, cold, or loses sensation.

Ask whether the nail bed or fingertip needs treatment, whether a hidden injury should be checked, and how to protect the nail while it grows. Ask which symptoms suggest infection and how quickly you should be reviewed if they appear.
