Tetanus shot after stepping on a nail

A nail can push bacteria and foreign material below the skin, even when the opening appears narrow. Foot punctures may also affect deeper structures or hide part of the object. Tetanus protection is judged from the wound type and your vaccination record, not from the appearance of rust. Do not assume a small surface mark means the injury is minor.

Rinse the area with clean running water, wash around it with soap and cover it with a clean dressing. Avoid digging into the wound or trying to close it yourself. Arrange assessment as soon as practical, particularly if the nail went through footwear, the puncture is deep, or dirt entered it. Bring any vaccination record and describe when the injury happened.

Get prompt care if pain, warmth, redness or swelling increases, drainage appears, fever develops, or you cannot bear weight or move your toes normally. Urgent assessment is needed for numbness, severe pain, a suspected retained fragment, or a wound over a joint. Jaw stiffness, swallowing difficulty, spasms or breathing problems require emergency help.
