What the evening pattern may mean

Tension-type pain is often pressing or tightening rather than throbbing. It may build through the day as the neck, scalp and jaw muscles remain tense, especially after screen use, prolonged sitting, driving or emotional strain. Fatigue, missed meals, too little fluid and an uncomfortable pillow can add to the discomfort. Night-time worsening does not by itself identify the cause or prove that the headache is a tension headache.

Notice whether the pain is on both sides, whether it changes with posture, and whether it comes with fever, vomiting, weakness, visual change or confusion. A headache diary can show links with sleep, caffeine, work tasks and pain-medicine use.

Rest in a quiet, comfortably dim room and drink water if you may be dehydrated. Gently relax the shoulders and jaw, apply warmth to tight neck muscles, and take a short break from close screens. Keep your head and neck supported without forcing them into one position. Use any over-the-counter pain medicine only as directed on its label and avoid relying on it repeatedly without advice, because frequent use can contribute to recurring headaches.

When to seek care

Seek urgent medical help for a sudden severe headache, headache after a head injury, new weakness or numbness, trouble speaking, fainting, confusion, seizure, persistent vomiting, fever with a stiff neck, or new vision loss. Arrange a GP assessment if night-time headaches keep returning, wake you from sleep, are progressively changing, or do not settle with usual self-care. A clinician can examine you and decide whether further evaluation is needed.
