Tension headache and pain behind the eyes

A tension-type headache is commonly described as pressure, tightness or aching rather than a throbbing pain. Tension in the forehead, scalp, jaw and neck can be felt around or behind the eyes. Screen work, poor posture, stress, missed meals and tiredness can make the discomfort more noticeable.

Pain that is severe in one eye, starts abruptly, follows an injury, or comes with a red eye, halos, vomiting, light sensitivity or reduced vision may have a different cause. The location of pain alone cannot identify the cause.

Pause close-up work, soften bright light and use a comfortable reading distance. Relax the jaw, lower the shoulders and try a warm compress over closed eyes if it feels soothing. Hydration, regular food and sleep can reduce common headache triggers. Use any over-the-counter medicine only as directed on its label.

Arrange a GP review if the pain keeps returning, lasts longer than expected, or is accompanied by changes in vision. A clinician can decide whether an eye examination or another assessment is appropriate.

Get urgent help for sudden severe headache, loss of vision, new double vision, a painful red eye, severe eye tenderness, weakness, confusion, difficulty speaking or fever with a stiff neck. Seek same-day advice for persistent pain behind one eye or repeated vomiting.

Ask whether the pain pattern is consistent with tension, whether your eyes need examination, and how to recognise a change that needs urgent review. Describe the side, duration, triggers and any visual symptoms.
