What should you know about a telescopic intramedullary nail?

The nail sits within the bone’s central canal and helps keep the bone aligned. A telescopic design has parts that can slide or extend as the limb grows, so it may reduce the need for repeated lengthening operations in carefully selected children. It is different from a standard fixed intramedullary nail, which is chosen mainly for stable fracture support. Imaging and growth patterns guide the decision.

Before surgery, discuss mobility at home, school or work needs, medicines, and any dental or skin infection. After surgery, protect the limb according to the weight-bearing plan and use crutches or other equipment correctly. Physiotherapy helps maintain nearby joint movement and rebuild strength. Keep scheduled imaging visits because the nail and bone must be checked as healing or growth changes the limb. Avoid contact activities until the treating team clears them.

Call the surgical team for increasing swelling, wound drainage, fever, pain that is worsening rather than settling, or a new change in limb alignment. Urgent assessment is needed for severe pain with a cold or pale foot or hand, new loss of movement or sensation, or sudden breathing difficulty. A fall or new impact should also be reported if it causes pain around the nail.

Ask which bone and section of the canal will receive the nail, how much weight the limb may carry, how growth will be monitored, and what could require adjustment or removal. Clarify the physiotherapy goals and the activities that are temporarily restricted.
