TCA Peel for Xanthelasma: What to Know

Xanthelasma consists of yellowish cholesterol-rich deposits, commonly near the inner eyelids. TCA chemically damages selected surface tissue, but the margin between the deposit and healthy eyelid skin is small. A misplaced or overly deep application can cause burns, scarring, pigment change or injury to the eye.

A clinician must first confirm that the lesion is xanthelasma and assess its depth, location, eyelid movement and skin characteristics. TCA may not suit thick, extensive or recurrent deposits. CO2 laser resurfacing or surgical excision may be discussed, with different healing patterns and risks.

Use a qualified medical provider experienced with periocular procedures. Share eye conditions, contact-lens use, medicines, previous eyelid procedures, healing problems and any history of raised scars. Do not use a home acid, cosmetic peel or unprescribed product on an eyelid.

Seek urgent eye care for blurred or reduced vision, severe light sensitivity, intense eye pain or suspected chemical entry. Breathing difficulty or sudden facial swelling is an emergency.
