TCA CROSS for ice pick scars: before and after

Before treatment, the clinician examines scar depth, width, skin tone, active acne and any tendency toward dark marks. During TCA CROSS, a small amount of trichloroacetic acid is placed inside selected scars rather than spread across the whole face. The controlled chemical injury starts a repair response that can build new collagen within the depression.

Immediately afterward, treated points may look frosted, red, swollen or darker. A small crust can form as the surface heals. These early changes are not the final result. Texture may soften over later weeks, and several appointments may be considered when scars remain visible. Ice pick scars can respond unevenly, and a scar may not disappear completely.

Tell the clinician about recent tanning, active breakouts, cold sores, eczema, keloid scarring, medicines and products that irritate your skin. Arrive with clean skin and follow the clinic’s instructions about pausing exfoliating acids or retinoids.

After treatment, keep the area clean, avoid picking crusts, and use only the aftercare product recommended for you. Protect the treated skin from sunlight with shade and suitable sunscreen once the clinician says it is appropriate; this helps reduce unwanted colour change. Avoid scrubs, waxing and unapproved home acids while the surface is healing.

Contact the treating clinic if pain, warmth, swelling or redness is increasing rather than settling, or if there is spreading discharge, a bad smell, fever, blistering or a wound that is not closing. Seek urgent medical help for facial swelling that affects breathing or vision. New dark or pale patches also deserve review so aftercare can be adjusted.
