What should you know about tamoxifen skin rash?

A tamoxifen-associated rash may be itchy, red, raised, or widespread, but appearance alone cannot confirm the cause. Consider the date treatment started, any recent dose change, and whether the rash followed a new soap, cream, antibiotic, or illness. Drug rash, contact dermatitis, heat rash, and other inflammatory or infectious conditions can overlap in appearance.

Write down whether the skin is painful, warm, blistered, peeling, or affecting the mouth or eyes. Photos taken from the same distance can help show change over time. The oncology team may coordinate with a dermatologist while protecting the treatment plan for the underlying condition.

Call the oncology team or prescribing clinician and describe when the rash started, how quickly it spread, and every medicine or skin product used recently. Use lukewarm water, a bland moisturiser, and loose clothing if these feel comfortable. Avoid adding several new creams because they can obscure the pattern or irritate the skin.

Do not stop tamoxifen, restart it, or substitute another medicine without the prescriber’s direction. Keep a list of treatments and bring photographs to the consultation.

Seek emergency help for breathing difficulty, throat or tongue swelling, faintness, or sudden facial swelling. Obtain urgent assessment for a rapidly spreading rash, skin pain, blisters, peeling, fever, or sores involving the mouth, eyes, or genitals. These signs can signal a serious reaction and require prompt examination.
