What tamoxifen means for bones

Tamoxifen acts on oestrogen receptors. In bone, its effect can vary with hormone status: it may help maintain bone density after menopause, while younger people can have different bone effects. Bone health also depends on age, previous fractures, family history, movement, nutrition, smoking, alcohol, and other medicines.

A bone-density scan may be considered when your overall risk makes it useful. The result is one part of the assessment; it does not decide treatment on its own. Do not stop tamoxifen because of a bone concern without speaking with the prescribing team.

Ways to support bone health

Take tamoxifen exactly as prescribed and tell your oncology team about steroid use, thyroid disease, kidney problems, prior fractures, or medicines that may affect bones. Regular weight-bearing activity and strength work can support muscles, balance, and bones when safe for you. Calcium-rich foods, adequate vitamin D, not smoking, and keeping alcohol low may also help.

Your clinician can decide whether you need a bone-density scan, blood tests, supplements, or a bone-directed medicine. Ask before starting supplements, because doses and interactions need to fit your health history.

When to seek care

Contact your care team promptly after a fall that causes lasting pain, new back or hip pain, loss of height, or a suspected fracture. Seek urgent help for severe pain after an injury, inability to stand or walk, or numbness or weakness in a limb. These symptoms can have causes other than tamoxifen, but they deserve assessment.
