What does a VDRL test show for syphilis?

VDRL stands for Venereal Disease Research Laboratory test. It detects antibodies that can appear when syphilis affects the body, rather than detecting the bacterium directly. A reactive result can support a syphilis diagnosis, but similar reactions can occur with other conditions, so another treponemal test is often used.

A non-reactive result does not always exclude recent infection. The result may also be interpreted alongside symptoms, exposure timing, previous treatment and other test findings. Clinicians may compare later results to assess response after treatment.

Ask the clinician who ordered the test to explain whether it was reactive or non-reactive and what confirmatory testing is needed. Mention any previous syphilis treatment, because antibodies and test patterns can persist or change after treatment.

If syphilis is diagnosed, follow the prescribed antibiotic plan and attend the recommended follow-up. Avoid sexual contact until you receive clear guidance, and make sure recent partners know they may need assessment. Do not self-treat based on a VDRL result alone.

Contact a clinician promptly after a reactive result, a known exposure, or symptoms such as a painless sore or unusual rash. Seek urgent care for eye pain or vision changes, hearing loss, severe headache, confusion or weakness. Pregnancy warrants prompt review after a reactive result or possible exposure.

Ask whether a treponemal confirmation test is needed, whether the result could reflect past treated infection, when repeat testing is appropriate, and how follow-up will be arranged. Ask when partners should be notified and when sexual contact can resume.
