What should you know about syphilis skin rash?

Syphilis is an infection that can move through stages. A later-stage rash may appear on the trunk, limbs, palms, or soles and may not be very itchy. Earlier infection can involve a painless sore that heals without treatment, while other people notice no obvious skin change. A rash can therefore be a presentation of syphilis, but its appearance cannot confirm the infection.

Many other conditions can cause a rash, including Atopic Dermatitis, Contact Dermatitis, Drug Rash, Heat Rash, Intertrigo, Psoriasis, Ringworm, or Scabies. The direction matters: a rash may occur with syphilis, but a rash does not by itself show that someone has syphilis.

Arrange prompt medical assessment for a new unexplained rash, especially after sexual contact with a new or untreated partner, a painless genital or mouth sore, swollen glands, or a partner's positive test. Testing may involve blood tests and an examination; tell the clinician about recent exposures, pregnancy, medicines, allergies, and previous treatment.

Avoid sexual contact until a clinician advises you about testing, treatment, and partner notification. Do not rely on a fading rash as proof that the infection has gone. Syphilis is treatable, but untreated infection can affect other organs, so follow-up instructions matter.

Seek urgent medical attention for vision changes, eye pain, hearing changes, severe headache, weakness, confusion, or difficulty walking with a suspected syphilis exposure or rash. Obtain prompt assessment for a rash during pregnancy, a rapidly spreading eruption, fever, or a painful skin reaction after starting medicine.
