Syphilis and light brown spots on the bottoms of the feet

A secondary syphilis rash can involve the palms and soles and may be subtle, reddish-brown, or rough rather than itchy. It can occur with swollen glands, tiredness, fever, mouth patches, or hair thinning, but symptoms vary and may be absent. Marks on the soles can also follow irritation, eczema, fungal infection, medication reactions, or normal variation in skin pigmentation. A photograph or description cannot establish the cause, and a syphilis rash may fade without treatment while infection remains.

Book an examination if the spots are new, spreading, persistent, or accompanied by a sore or recent sexual exposure. A clinician can inspect both feet and other skin areas, ask about timing and contact, and arrange a syphilis blood test when appropriate. Avoid sexual contact until you have advice if syphilis is possible. Do not apply strong steroid or bleaching products before assessment unless prescribed, because they can alter the appearance.

Seek prompt care for new spots with a painless mouth, genital, or anal sore, swollen glands, fever, or a rash on the palms. Get urgent help for eye pain, vision change, hearing change, severe headache, confusion, weakness, or difficulty walking. Pregnancy with a possible exposure requires prompt clinical advice, even when the skin marks are mild.

Ask whether the pattern fits an infectious rash, whether you need syphilis and other skin or sexual-health tests, and whether a repeat test is needed after recent exposure. Ask what treatment is suitable and how to protect partners while results are being clarified.
