Syphilis in the throat

A sore in the mouth or throat may be painless, and some people notice no local symptoms. Other possible signs include a rash, swollen glands, fever, or a sore throat, but these features overlap with many common conditions. Oral exposure can transmit syphilis in either direction when an infectious sore is present. A clinician may combine an examination with blood testing and other tests suited to the site and timing of exposure.

Arrange STI screening if you have had oral sexual contact with a partner who has syphilis, has symptoms, or whose status is unknown after a concerning exposure. Tell the clinician about mouth or throat symptoms and when the contact occurred. If diagnosed, complete prescribed antibiotics, avoid sexual contact until advised, and ensure partners are contacted for testing and treatment. Do not share medication or rely on mouthwash to treat the infection.

Seek prompt assessment for a mouth or throat sore that persists, a spreading rash, trouble swallowing, breathing difficulty, or swelling of the neck. Vision changes, hearing changes, severe headache, weakness, or confusion need urgent medical attention because they can signal involvement beyond the throat.

Ask which tests match an oral exposure, whether testing should be repeated, whether other STI screening is appropriate, and which symptoms should trigger urgent review. Ask when kissing or sexual contact can resume after treatment.
