It commonly spreads through direct contact with an infectious sore during sexual contact. A sore may be painless and easy to miss. Later signs can include a rash, swollen glands, fever, or tiredness, while some people have no noticeable symptoms. Without treatment, the infection can remain in the body and may cause serious problems affecting the brain, nerves, eyes, or heart.

Arrange a clinical assessment and testing if you have symptoms or a recent sexual exposure. Tell the clinician about the timing of exposure, any medicines or allergies, and whether pregnancy is possible. If testing confirms syphilis, complete the prescribed antibiotic plan, avoid sexual contact until advised, and make sure partners are offered testing and treatment. A broader STI screening assessment may also be useful.

Get prompt care for a new eye symptom, hearing change, severe headache, weakness, confusion, or difficulty walking. A genital, oral, or anal sore, or a widespread rash after sexual contact, also deserves timely assessment even when it is not painful.
