What should you know about syphilis bumps on the tongue?

Syphilis may cause a painless sore in the mouth during an early stage, and later illness can include mouth or throat changes. The appearance alone cannot identify syphilis. Irritation, an inflamed taste bud, a viral illness, thrush, dental trauma, or another mouth condition may look similar. A syphilis infection can also be present without visible oral changes.

A clinician may inspect your tongue, mouth, throat, skin, and lymph nodes, then offer a blood test and other tests when appropriate. Tell them about oral sex, kissing with a mouth sore, genital or skin symptoms, and the timing of possible exposure. This information helps decide which tests and follow-up are useful.

Arrange an STI assessment if the bumps persist, are painless, follow a possible exposure, or occur with a rash, swollen glands, or a genital sore. Avoid picking or irritating the area, and avoid sexual contact that could expose another person until you have medical advice. Do not apply antibiotics or strong oral medicines without guidance.

If syphilis is diagnosed, take the prescribed antibiotic exactly as directed and attend follow-up. Recent partners may need confidential assessment. A sore that improves does not necessarily mean the infection has gone, so do not skip testing or review.

Seek prompt care for a tongue or mouth lesion with trouble breathing or swallowing, rapidly increasing swelling, heavy bleeding, severe pain, or dehydration. Arrange timely assessment for a persistent painless sore, new rash, vision change, severe headache, or a known syphilis exposure. Urgent review is especially important during pregnancy.

Ask what the tongue changes resemble, whether STI screening is appropriate, which samples will be tested, whether you should avoid kissing or sex while waiting, how partners are handled, and when a persistent lesion needs dental or specialist review.
