What the swelling may mean
Late in pregnancy, the body holds more fluid. The growing uterus can also press on veins that return blood from the legs, so fluid may collect around the feet and ankles. Swelling can be more noticeable at the end of the day, after standing, or in warm weather, and it may improve after resting with the legs raised. Mild swelling that develops gradually in both feet is often part of normal late-pregnancy changes.
Swelling alone cannot identify its cause. Swelling of the face or hands, a sudden increase in swelling, or swelling together with headache, changes in vision, pain below the ribs, nausea, or feeling unusually unwell can occur with pre-eclampsia, a pregnancy complication that needs medical review.
What you can do for comfort
Change position regularly and avoid standing for long periods. Rest on your side, preferably the left side if that is comfortable, and place your feet on a cushion when sitting. Wear supportive, comfortable shoes and avoid tight socks or straps around swollen areas. Gentle ankle movements, such as flexing and circling the feet, can help you stay comfortable. Keep drinking fluids as usual; do not sharply restrict fluids or take water tablets unless your maternity clinician prescribes them. Mention persistent swelling at your next antenatal appointment so your blood pressure and urine can be checked.
When to seek care
Contact your maternity unit or clinician promptly if swelling appears suddenly, becomes pronounced, or affects your face or hands. Seek urgent advice for a severe or persistent headache, blurred vision or flashing lights, pain under the ribs, vomiting, shortness of breath, or a strong sense that something is wrong. One leg that becomes much more swollen, painful, red, or warm than the other also needs urgent assessment because it may signal a blood clot. Do not wait for a routine appointment when these warning signs occur.
