Short answer

Swollen feet after pregnancy can result from retained fluid and recovery after birth, but sudden or worsening swelling with headache, breathing trouble, or one-sided leg pain needs urgent care.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common setting
Puffiness in both feet or ankles during early recovery after birth.
Urgent change
One-sided painful swelling or swelling with headache, vision change, or breathlessness.

Understanding postpartum puffiness

Fluid collected during pregnancy and birth does not leave the body immediately. Hormonal changes, reduced movement, intravenous fluids, and the effort of labour can all contribute to puffy feet or ankles after delivery. Swelling may affect both sides and feel tight by the end of the day. Postpartum complications can also appear after birth, so a new symptom should be considered with blood pressure, pain, bleeding, temperature, and breathing rather than judged by appearance alone.

Steps that may help

Rest with your feet raised, change position regularly, and take gentle walks if your maternity team says activity is safe. Wear roomy supportive footwear and avoid tight socks. Keep drinking normally, especially if breastfeeding, unless a clinician has given different instructions. Do not take water tablets or anti-inflammatory medicines without checking that they are appropriate after birth and during breastfeeding. Tell your postnatal team if swelling is not easing or is making movement difficult.

Postpartum warning signs

Seek urgent medical help for sudden facial or hand swelling, a severe headache, blurred vision, upper abdominal pain, chest pain, severe breathlessness, fainting, or feeling very unwell. One leg that becomes more swollen, painful, warm, red, or tender needs urgent assessment. These symptoms can signal pre-eclampsia after birth, a blood clot, or another complication. Do not wait for a routine postnatal check when these features occur.

Questions after delivery

Ask whether your blood pressure should be checked, whether the swelling fits your birth and recovery, and which symptoms require emergency care. Mention any intravenous treatment, medicines, reduced mobility, headache, vision change, or one-sided pain.

Questions people ask

How should I monitor swelling after birth?

Notice whether it affects one or both sides, whether it is increasing, and whether pain, headache, vision change, or breathing symptoms appear.

Can postpartum pre-eclampsia happen after delivery?

Yes. Warning symptoms after birth need prompt assessment rather than being attributed automatically to normal fluid changes.
