Swelling in pregnancy: when to worry

Gradual puffiness in both ankles can develop as pregnancy affects fluid balance and slows circulation from the legs. A concerning pattern is different: swelling may spread quickly, involve the face or hands, or fail to improve with rest. One painful, warm, red, or tender leg may point to a circulation problem rather than ordinary fluid retention. Swelling does not diagnose a condition on its own, so clinicians use your symptoms, blood pressure, urine, and examination together.

If the change is mild but new, contact your maternity team for advice and describe when it began, where it is, and whether it is worsening. Rest with your legs raised, keep gently mobile, and remove tight footwear or rings. Do not massage a painful one-sided leg or take medication for fluid loss without medical advice. Keep a list of headache, vision, breathing, or abdominal symptoms so you can report them clearly.

Seek urgent maternity or emergency assessment for sudden swelling with a severe headache, blurred vision, flashing lights, pain beneath the ribs, vomiting, or feeling very unwell. Chest pain, severe breathlessness, fainting, or a painful swollen leg requires emergency help. These warning signs can occur with serious problems such as pre-eclampsia or a blood clot and should not wait for a routine appointment.

Ask whether you need a same-day blood-pressure and urine check, how to monitor the swelling, and which number or service to call outside clinic hours. Ask whether your medicines, activity, or another health issue could affect the pattern.
