Swelling in the lower abdomen during pregnancy

The uterus expands through pregnancy, changing the shape and tension of the lower abdomen. Bloating, constipation, stretched muscles, and pressure from the growing pregnancy can also make the area feel full or tight. A gradual change without severe pain is often different from a sudden, tender swelling.

Describe exactly where the change is, whether it is soft or firm, and whether it varies with meals, bowel movements, position, or activity. Pre-eclampsia more often involves swelling elsewhere and blood-pressure-related symptoms, so lower abdominal fullness alone does not establish that diagnosis.

Wear loose clothing, change position gently, and take comfortable walks if your clinician allows. Regular meals, adequate usual fluids, and fibre-rich foods may help if constipation or bloating is contributing. Avoid pressing repeatedly on the area or taking laxatives and pain medicines without pregnancy-specific advice.

Call your maternity team promptly for persistent or worsening pain, a firm tender area, vaginal bleeding, fluid leakage, fever, vomiting, painful urination, reduced fetal movement when movement has been established, or sudden swelling with severe headache or visual symptoms.
