What should you know about swelling in hands and feet after birth?

Pregnancy-related fluid and the changes around delivery may leave the hands, fingers, ankles or feet puffy for a while. Swelling can be more noticeable after standing or late in the day and may affect both sides. It should gradually improve rather than steadily intensify. A painful, hot or one-sided area has a different significance and should not be explained away as routine postpartum swelling. A history of rheumatoid arthritis or a recent wrist injury can also affect hand symptoms and should be mentioned.

Rest with the hands or feet raised when possible, change position regularly, and take gentle walks if your maternity team has said activity is safe. Wear comfortable footwear and remove tight rings. Keep fluids to normal thirst and follow your discharge instructions; do not start water tablets or herbal remedies on your own. Check whether swelling is easing, and record associated headache, blood-pressure readings if you were asked to monitor them, pain, redness or shortness of breath.

Contact your maternity team promptly for sudden facial or hand swelling, a severe or persistent headache, visual changes, upper abdominal pain, nausea with feeling unwell, or a raised blood-pressure reading. Get emergency help for chest pain, breathlessness, fainting, confusion, or a painful swollen leg. Fever, spreading redness or drainage also needs prompt review. These symptoms can occur after delivery and should be assessed even if pregnancy blood pressure was normal.

Ask how long your pattern should be observed, whether your blood pressure or urine needs checking, and which pain medicines are suitable while breastfeeding. Tell the clinician when the swelling began, whether it is equal on both sides, and what other symptoms appeared.
