Understanding the location

The back of the hand contains small joints, tendons, soft tissue, and blood vessels, so swelling there does not identify one cause by itself. A knock, fall, or overuse can produce tenderness and puffiness. A wrist injury, including a distal radius fracture, may make the back of the hand swell even when the wrist is the main source. Rheumatoid arthritis commonly presents with swollen, tender joints and stiffness, sometimes on both sides. A soft lump, warmth, redness, bruising, reduced grip, or tingling changes how quickly it should be checked.

Helpful first steps

Take off rings and bracelets before swelling increases. Rest from gripping, lifting, and repetitive wrist work, while gently opening and closing the fingers if this does not increase pain. Elevate the hand during rest. After a minor recent injury, use a cool pack wrapped in cloth for brief periods and protect the skin. Avoid heat or vigorous massage while the cause is uncertain, particularly if the area is hot or newly injured. Arrange a review if swelling persists, returns, or interferes with writing, dressing, or gripping.

Seek urgent assessment for a visibly deformed hand or wrist, severe pain after a fall, an open wound, or inability to move the fingers. Prompt care is also needed for rapidly increasing swelling, fever with hot red skin, new numbness, or fingers that become pale, blue, or unusually cold. If symptoms are milder but do not settle, a clinician can examine the joints and tendons and decide whether imaging is useful.
