Swelling hands in old age: what to know

Swelling may affect one hand or both. Rings can feel tight, finger movement may become stiff, and the skin can look shiny. Arthritis can irritate the joints; rheumatoid arthritis commonly presents with painful, swollen joints and morning stiffness. A fall can injure the wrist or hand, including a distal radius fracture, which may cause swelling, bruising, tenderness, or difficulty using the hand. Heat, redness, fever, numbness, or colour change gives additional clues. Swelling in both hands may also be related to fluid balance or medicines, so the wider health picture matters.

Remove tight rings early and rest the hand from activities that increase discomfort. Keep it gently moving within a comfortable range so stiffness does not build. Elevating the hand above heart level for short periods may ease dependent swelling. A wrapped cool pack can help after a minor strain; protect the skin and stop if it worsens symptoms. Do not tightly massage a newly injured or very painful area. Arrange a clinical review if swelling keeps returning, limits grip, or has no clear explanation. Bring a list of medicines and note when the swelling appears.

Seek urgent care after a significant fall, visible deformity, severe pain, or inability to move the fingers or wrist. Get prompt medical help for a hand that becomes hot and red with fever, or for sudden swelling with blue or pale fingers, new numbness, or marked weakness. Contact a clinician soon if both hands remain swollen, the swelling is gradually increasing, or morning stiffness and joint pain continue. These patterns need examination rather than self-treatment alone.

Ask whether the pattern fits joint inflammation, an injury, fluid retention, or a medicine effect. Ask which examination or imaging would clarify the cause, whether any medicines should be reviewed, and which hand movements are appropriate while symptoms settle.
