Swelling between the knuckles on top of the hand

The back of the hand contains tendons, joints, small blood vessels and soft tissue that can all look puffy when irritated. Recent impact, a fall, repetitive gripping or a tight ring can cause local swelling. Stiffness in several finger joints, especially with swelling on both sides or morning difficulty moving, can be a presentation of rheumatoid arthritis. Swelling after a wrist injury can occur alongside a distal radius fracture, even when the most noticeable change is in the hand. Color change, warmth, bruising, numbness or reduced movement adds useful context.

Remove rings and watches early if the hand is enlarging. Rest from forceful gripping, keep the hand raised when practical, and use a wrapped cool pack for short periods if the swelling followed activity or a minor knock. Keep the fingers gently moving within comfort rather than holding them rigid. Do not massage a newly injured or very tender area. Arrange an assessment if swelling persists, returns, or interferes with grip; a clinician may examine the joints, tendons, circulation and sensation.

Seek urgent care for a visibly deformed hand, severe pain after an injury, rapidly increasing swelling, pale or blue fingers, new numbness, or inability to move a finger. Prompt medical review is also appropriate when the area is hot and red with fever, or when swelling affects several joints and daily tasks. These features need examination because they can signal a fracture, infection, or significant inflammation.

Ask which structure appears swollen, whether an X-ray or other test is needed, and which hand movements are safe while it settles. Mention the onset, any injury or repetitive activity, stiffness on waking, skin changes, and medicines or conditions that may affect swelling.
