What should you know about swelling ankles and feet during pregnancy?

Pregnancy changes fluid balance and can make the growing uterus press on veins that return blood from the legs. Swelling may be more noticeable later in the day, after standing, or in warm weather, and it often affects both sides. Swelling alone does not identify a skin condition. If an itchy rash appears as well, its pattern and timing may help a clinician distinguish Atopic Dermatitis, Contact Dermatitis, Hives or another cause.

Rest with your legs supported when practical, avoid standing still for long periods, and make gentle movement part of the day if your maternity team says it is safe. Wear comfortable footwear and avoid tight bands around the ankles. Do not start medicines, compression garments or herbal products without checking that they are suitable for your pregnancy. Note whether swelling is equal on both sides and when it changes.

Contact your maternity team promptly for sudden swelling of the face or hands, a severe headache, vision changes, pain beneath the ribs, unusual shortness of breath or feeling very unwell. One-sided leg swelling with calf pain, warmth or redness also needs urgent assessment. Emergency help is needed for chest pain, severe breathing difficulty or collapse.

Ask whether the swelling pattern is reassuring, how to monitor blood pressure and warning symptoms, and whether an accompanying rash should be checked for Atopic Dermatitis, Contact Dermatitis or Hives.
