What may be happening

A sprained ankle damages supporting ligaments and can cause bruising as small blood vessels leak into nearby tissue. A fracture may produce similar swelling but often causes marked tenderness over bone and difficulty standing. Swelling can spread into the foot as fluid moves downward with gravity. Ankle osteoarthritis may cause activity-related puffiness without a single injury, while gout can bring sudden heat, redness, and intense joint pain. The colour of a bruise does not show how serious the injury is by itself.

Early care

Protect the ankle from movements that increase pain and walk only as much as feels safe. Elevate the foot, use a cold pack wrapped in cloth for short periods, and keep the ankle support fitted as instructed. Gentle movement may help stiffness when pain allows, but do not force the joint or return to sport while walking remains painful. A clinician can decide whether an X-ray or rehabilitation plan is needed.

When to seek care

Seek prompt assessment after a major twist, a direct blow, worsening swelling, or inability to take a few steps. Get urgent help for a deformed ankle, an open wound, numb or cold toes, blue or pale skin, severe pain, or rapidly increasing tightness. Fever with a hot red ankle also needs urgent medical review.
