What should you know about swab test for water breaking?

When the waters may have broken, a clinician can examine the fluid and use a sterile swab to collect a sample from the vagina. The test looks for chemical or physical features associated with amniotic fluid. The result is interpreted alongside your symptoms, pregnancy stage, examination findings and the baby’s wellbeing; a swab is not the only part of the assessment.

Amniotic fluid may appear as a sudden gush or a continuing trickle, while urine or normal vaginal discharge can look similar. Avoid putting anything in the vagina before assessment unless your maternity team tells you to, because this may affect examination and infection precautions.

Contact your maternity unit for advice rather than waiting for the leak to stop. Note when it began, its colour and smell, and whether it keeps wetting a pad. Use a clean maternity pad, not a tampon, and take your pregnancy records if you attend. The team may also check temperature, contractions, fetal heart rate and the cervix.

Seek urgent maternity assessment for suspected leaking fluid, especially with fever, abdominal pain, regular contractions, vaginal bleeding, reduced fetal movement, green or brown fluid, or an unpleasant smell. Do not rely on a home swab or a reassuring result if fluid continues to leak or you feel unwell.
