How symptoms can present

Food poisoning after sushi can result from contaminated ingredients, unsafe handling, poor temperature control or toxins produced before the food was eaten. Symptoms may begin with nausea, cramping or vomiting, followed by diarrhoea and sometimes fever. The timing and combination vary with the cause, so symptoms cannot identify a particular germ by themselves. Fish and other ingredients can also cause allergic reactions, which may involve hives, swelling or breathing difficulty rather than a stomach illness.

The main early concern is fluid loss. Vomiting and diarrhoea can make it difficult to replace water and salts, leading to thirst, dizziness, dry mouth, reduced urination or unusual weakness.

What to do at home

Rest and take frequent small sips of water or an oral rehydration solution. If vomiting is present, pause briefly and restart with smaller sips rather than drinking a large amount at once. When appetite returns, choose simple foods and avoid alcohol or foods that worsen symptoms. Wash hands carefully and avoid preparing food for others while diarrhoea or vomiting continues. Oral Rehydration Therapy can help replace fluid and salts; IV Fluid Therapy may be considered by clinicians when drinking is not possible or dehydration is significant.

When to seek care

Seek urgent help for severe abdominal pain, blood in vomit or stool, fainting, confusion, difficulty breathing, swelling of the lips or tongue, or inability to keep fluids down. Arrange medical assessment if fever is high or persistent, diarrhoea or vomiting is worsening, or signs of dehydration continue. Babies, older adults, pregnant people and those with significant medical conditions may need earlier advice because fluid loss can become serious more quickly.
