What should you know about surgery for ovarian cancer stage 4?

Stage 4 means ovarian cancer has spread beyond the pelvis to distant areas or organs. Surgery may be considered before or after chemotherapy, depending on where the cancer is, whether it can be removed safely, and how well your body could tolerate a major operation.

An operation may involve removing the ovaries and fallopian tubes, the uterus, or affected tissue elsewhere in the abdomen. Debulking surgery aims to remove as much visible cancer as safely possible; it is not suitable for every person. Imaging, examination, and sometimes genomic tumour profiling help shape the plan.

Ask the gynaecological oncology team to explain the goal of surgery, the areas they expect to treat, and whether chemotherapy is planned before or after it. Discuss recovery, nutrition, physical activity, bowel or urinary changes, and possible effects on fertility or menopause. Bring a list of medicines and other health conditions.

Your plan may include pelvic ultrasound or other imaging, tissue testing, and genetic assessment such as BRCA testing. Chemotherapy or palliative care can be part of care at the same time; palliative care focuses on symptoms and quality of life and does not mean that other treatment must stop.

After surgery, seek urgent advice for fever, worsening abdominal pain, repeated vomiting, a swollen or hard abdomen, heavy bleeding, wound separation, or redness with drainage. New breathlessness, chest pain, fainting, or one-sided leg swelling needs emergency assessment. Before treatment, contact the team quickly if pain, vomiting, or inability to pass urine or stool is worsening.

Ask what surgery can realistically achieve, whether an operation is recommended now, and how chemotherapy affects timing. Ask which organs may be removed, how a bowel or urinary change would be managed, what recovery support is available, and how symptoms will be treated if surgery is not suitable.
