What should you know about suprapatellar plica syndrome?

Short answer
Suprapatellar plica syndrome is irritation of a fold in the knee lining above the kneecap, often causing pain with bending, clicking, or a catching sensation.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer
Suprapatellar plica syndrome is irritation of a fold in the knee lining above the kneecap, often causing pain with bending, clicking, or a catching sensation. The fold, called a plica, is normal tissue in many knees; repeated bending, a sudden increase in activity, or an injury can make it thickened and sensitive. Symptoms can overlap with other knee problems, so an examination is useful when discomfort continues.
What suprapatellar plica syndrome means
The suprapatellar plica lies between the front of the thighbone and the tissues around the kneecap. Irritation may produce pain above or beside the kneecap, especially when climbing stairs, squatting, cycling, running, or standing after sitting. Some people feel a soft snap or a band moving as the knee bends. Swelling may be mild, and the knee can remain stable. Knee clicking alone does not establish the diagnosis, while true locking or inability to straighten the knee needs separate assessment.
A clinician may ask when the symptoms began, examine kneecap movement and tenderness, and compare both knees. Scans are not always needed, but imaging can help look for cartilage, tendon, ligament, or meniscus conditions when the findings are unclear.
What you can do
Reduce or pause the movements that repeatedly trigger the pain, while keeping comfortable movement within your limits. Ice wrapped in a cloth for short periods may ease soreness after activity. A physiotherapist can guide gradual exercises for thigh and hip strength, flexibility, movement control, and a safe return to sport or work. Progress should be based on how the knee responds during the activity and later that day.
Pain-relieving or anti-inflammatory medicine may not suit everyone, particularly people with stomach, kidney, heart, bleeding, pregnancy, or medicine-related concerns. Ask a pharmacist or clinician which option is appropriate for you. If symptoms persist despite a guided rehabilitation plan, an orthopaedic clinician can discuss further options; keyhole surgery is considered only in selected cases.
When to seek care
Arrange an assessment if knee pain keeps returning, limits stairs or exercise, or is accompanied by repeated catching or swelling. Seek urgent medical care after a major injury if the knee looks deformed, becomes rapidly swollen, cannot bear weight, or cannot fully straighten. Prompt care is also appropriate for a hot, red knee with fever, or severe pain with feeling unwell, because these features may point to a problem other than plica irritation.
Questions for your doctor or physiotherapist
You could ask: What findings support plica irritation in my knee? Which activities should I modify, and which exercises are suitable now? How will we measure improvement? Do I need imaging to exclude another cause? If rehabilitation does not settle the symptoms, what would you recommend next?

Questions people ask
Can suprapatellar plica syndrome cause knee clicking?
Yes. An irritated plica can create a snap or click as it moves during knee bending, but clicking can have other causes and should be interpreted with the rest of the examination.
Is surgery always needed for suprapatellar plica syndrome?
No. Many people begin with activity changes and physiotherapy. Knee arthroscopy may be discussed when persistent symptoms have not improved with appropriate non-surgical care and another cause has been considered.
