Sudden sweating, shaking and feeling faint can happen when your blood sugar, blood pressure, body temperature or stress response changes quickly; sit or lie down, and seek urgent help if severe warning signs occur. These symptoms can settle after rest, fluids or food, but a new, repeated or unexplained episode deserves medical assessment. Feeling faint means you may feel lightheaded, weak or as though you might pass out; it is different from the room-spinning sensation of vertigo, although the sensations can overlap.

What these symptoms may mean

The combination may follow missed food, vomiting, heavy exertion, standing suddenly, a hot environment or not drinking enough. Hypoglycaemia can cause shaking, sweating, weakness, confusion or a fast heartbeat, particularly in someone using diabetes medication. Dehydration and heat exhaustion can reduce the amount of fluid circulating in the body and make lightheadedness worse.

A strong fear response or panic can produce sweating, trembling, rapid breathing and lightheadedness. Anaemia, including iron deficiency anaemia, can contribute to tiredness and dizziness because the blood carries less oxygen. A heart rhythm problem such as atrial fibrillation may cause palpitations, breathlessness, weakness or dizziness. Position-related spinning is more suggestive of benign paroxysmal positional vertigo than of simple faintness. These are possibilities, not a diagnosis based on symptoms alone.

What to do during an episode

Stop what you are doing and sit or lie flat straight away. If possible, raise your legs, loosen tight clothing and ask someone to stay with you. Move to a cool place if heat may be involved, and take small sips of water when you are fully alert and able to swallow. Do not drive, climb, swim or operate machinery while you feel faint.

If you have diabetes and can safely check your glucose, follow your personal hypo plan. Do not give food or drink to someone who is drowsy, unconscious or having a seizure. Note what you were doing, whether you had eaten, any medicines taken, how long the episode lasted and whether your heartbeat felt irregular; this information can help a clinician identify a pattern.

When to seek care

Call emergency services now if faintness occurs with chest pain, severe breathlessness, a new irregular or very fast heartbeat, sudden weakness on one side, trouble speaking, a severe headache, a seizure, heavy bleeding or a serious injury. Get immediate help if the person becomes unconscious, does not recover promptly, or cannot swallow safely.

Arrange prompt medical advice when episodes recur, happen during exercise, follow a new medicine, involve an actual blackout, or occur with pregnancy, diabetes or known heart disease. A clinician may check your blood pressure and pulse, review medicines and eating or drinking patterns, and decide whether blood tests or a heart tracing are appropriate.
