What should you know about submental cellulitis?

The submental area is the soft tissue below the chin. Cellulitis develops when bacteria enter through a break in the skin, such as a scratch, inflamed hair follicle, wound, insect bite, or another nearby skin problem. The affected area may become red, warm, tender, and swollen; the edges of redness may spread over time. Fever, chills, tiredness, or swollen glands can accompany the local infection.

A clinician may examine the skin, mouth, teeth, and neck to look for the source and to judge whether the infection is superficial or reaching deeper spaces. Dental infection, salivary-gland problems, or a collection of pus can cause similar swelling and may need different treatment. Type 2 diabetes and conditions that weaken immune defenses can make infection harder to control.

Arrange prompt medical care rather than trying to treat expanding redness or swelling at home. Treatment commonly involves an antibiotic selected for the suspected bacteria and the infection's severity. Take it exactly as prescribed and complete the course unless the prescriber changes the plan. If a pocket of pus is present, drainage may be needed. Keep the area clean, avoid squeezing or puncturing it, and note whether the redness, warmth, pain, or swelling is changing.

Seek emergency help now for trouble breathing, swallowing, speaking, or opening the mouth; drooling; a muffled voice; rapidly increasing swelling under the chin or tongue; severe neck stiffness; confusion; faintness; or marked weakness. These signs can indicate deeper spread or pressure near the airway. A high or persistent fever, shaking chills, red streaking, or feeling suddenly much worse also needs urgent assessment because a serious bloodstream infection can develop.

If symptoms are milder but the area is warm, painful, red, and enlarging, contact a clinician promptly. Do not wait for the skin to become dramatically swollen before arranging care.
