Recognising the pattern
A sore throat is a common presentation of strep throat, but a sore throat alone does not establish the diagnosis. Common cold, influenza, COVID-19, glandular fever, allergic rhinitis and acid reflux can also irritate the throat. The combination of fever, tender swollen glands, tonsil changes and absence of cough may make bacterial infection more worth assessing, without proving it.

Rest, take regular fluids, and choose soft foods if swallowing is uncomfortable. Use suitable pain relief according to the label and your clinician's advice. Do not start leftover antibiotics or share them. If strep throat is confirmed or strongly suspected, follow the prescribed course and ask when it is safe to return to school or work. Seek urgent care for breathing difficulty, drooling because swallowing is impossible, rapidly worsening neck or throat swelling, severe one-sided pain, confusion, or inability to drink. Arrange a prompt appointment for persistent fever, worsening pain, a rash, repeated episodes, or symptoms in a person with a weakened immune system. Children who are unusually sleepy or not passing urine need prompt attention.
