Strep throat without fever: what to know

Strep throat may cause a sudden sore throat, pain when swallowing, red or swollen throat tissue and tender swollen glands. Fever can be absent, especially early in the illness or in some people. A cough, runny nose or hoarse voice may point more toward a viral cause, but no single symptom is decisive. A clinician can consider the full pattern and examine your throat; a swab may be needed when the diagnosis is uncertain.

Keep drinking, even if you need small frequent sips, and select soft foods until swallowing is easier. Rest and avoid sharing cups or utensils while you have an infectious sore throat. Do not start antibiotics solely because the throat is painful, and do not dismiss the illness solely because there is no fever. Book a GP consultation if symptoms are significant, persistent or difficult to interpret.

Seek prompt advice if throat pain is getting worse, one side becomes much more swollen, or you are unable to eat and drink adequately. Emergency help is needed for breathing difficulty, drooling because swallowing is not possible, severe weakness, or a rapidly changing voice. These warning signs matter even when your temperature is normal.
