Strep throat not improving after three days

A sore throat can take time to settle, and the original diagnosis may need checking because viral illness, tonsillitis, or another throat problem can look similar. If antibiotics were prescribed, missed doses, vomiting, difficulty swallowing, or an organism that is not covered may affect the response. Persistent pain alone does not show that treatment has failed, but a clear deterioration, new swelling, or inability to drink needs prompt attention. Keep the medicine list and the timing of each dose ready for your clinician.

Contact the prescriber or arrange a GP consultation for advice rather than changing, doubling, or extending antibiotics yourself. Take prescribed medicine exactly as directed unless a clinician tells you otherwise. Continue fluids, soft foods, rest, and suitable pain relief. Check whether you can swallow enough to stay hydrated and whether urine is becoming unusually dark or infrequent. Avoid sharing antibiotics and replace your toothbrush only if your clinician advises it; the key issue is a fresh assessment of the diagnosis and response.

Seek urgent care if breathing is difficult, saliva cannot be swallowed, drooling occurs, the voice becomes muffled, or swelling is rapidly increasing. One-sided throat pain with difficulty opening the mouth can also need urgent assessment. Get prompt medical advice for dehydration, a spreading rash, marked weakness, or a new or persistent fever. Children and people with reduced immunity should use a lower threshold for contacting a clinician.
