Strep throat without fever: what to know

Fever is one possible feature of strep throat, not a requirement. People may instead notice a painful sore throat, red or swollen tonsils, white patches, or swollen glands in the neck. A lack of fever does not establish that the infection is bacterial, and it does not rule it out. Viral infections, tonsillitis, allergies, reflux, and dry air can cause similar throat symptoms. A clinician may use your symptoms, an examination, and a throat test when appropriate.

Keep drinking fluids and choose foods that are easy to swallow. Rest your voice, avoid smoke, and wash your hands after coughing or touching your mouth. A suitable pain reliever may help; follow the product directions and check with a pharmacist if you take other medicines or have a health condition. Book a GP consultation if the sore throat is severe, lasts longer than expected, keeps returning, or is accompanied by swollen glands or visible tonsil changes. Do not start antibiotics without medical advice.

Get urgent care for trouble breathing, inability to swallow liquids or saliva, drooling, rapidly increasing neck or throat swelling, or a muffled voice. Seek prompt advice if pain is strongly one-sided, you cannot keep fluids down, you feel faint, or a rash appears. Fever is not needed for these warning signs. If symptoms are mild but do not improve, arrange a review so another cause can be considered.
