What should you know about strep rash on feet?

A strep-associated rash is often described as fine and rough and may occur with fever, sore throat, painful swallowing, or tender neck glands. Feet can also develop redness, scaling, cracks, or blisters from sweat, friction, footwear, eczema, contact irritation, or ringworm. Itch, burning, tenderness, and whether one or both feet are affected can help a clinician narrow the possibilities. A strep infection may present with a rash, but the presence of a foot rash does not show that strep is responsible. The skin and any throat symptoms should be assessed together.

Arrange medical advice if a new foot rash appears with fever or sore throat. Keep feet clean and dry, change damp socks, choose roomy footwear, and avoid picking at cracks or blisters. Do not share socks, towels, or shoes while an infectious cause is possible. Avoid applying multiple treatments at once because this can irritate skin and obscure its appearance. Note recent shoes, creams, medicines, swimming, and whether the rash is between the toes. Ringworm, Contact Dermatitis, and Atopic Dermatitis may need different treatment from a strep-related illness.

Seek urgent care for rapidly increasing swelling, severe pain, spreading redness, warmth with fever, pus, red streaks, purple spots, numbness, or skin that turns pale or dark. Breathing difficulty, facial swelling, widespread blistering, or inability to drink requires immediate help. People with diabetes, poor circulation, or reduced immunity should seek advice promptly for a painful or infected-looking foot rash.

Ask whether the feet need a separate skin examination, whether throat testing is indicated, how to protect others, and which cleansing or moisturising products are appropriate. Mention pain, itch, drainage, new footwear, medicines, and allergies.
