Stomach pain during the fertile window

Ovulation can cause one-sided lower-abdominal discomfort around the middle of a cycle, sometimes with a small amount of spotting or increased discharge. Pain that is severe, lasts longer than expected or occurs with fever, vomiting or abnormal bleeding needs another explanation considered. Endometriosis, ovarian cysts, pelvic infection, urinary problems and pregnancy-related conditions can overlap with ovulation pain. Digestive problems such as gastroenteritis, Crohn's disease or diverticular disease may follow their own pattern and are not diagnosed from cycle timing alone.

Rest, use a warm pack if comfortable and note the cycle day, side and duration of pain. Record bleeding, discharge, urinary symptoms, bowel changes and any chance of pregnancy. Simple pain relief may help when it is suitable for you; check with a pharmacist if you have stomach, kidney or liver disease or take other medicines. If pain keeps returning each cycle, book a GP or sexual-health appointment to discuss examination and possible testing.

Get urgent help for sudden severe one-sided pain, fainting, shoulder-tip pain, heavy bleeding, fever with marked illness or persistent vomiting, especially if pregnancy is possible. Seek prompt assessment for pain with unusual discharge, pain during sex, difficulty urinating or a swollen abdomen. These features can indicate a pelvic or abdominal problem needing treatment rather than routine cycle discomfort.
